What is insured within CM-MediKo Plan?

CM-MediKo Plan provides extensive coverage for:

Reimbursement of co‑payments: up to €1500 per year

If you visit a general practitioner, specialist or another healthcare provider, you receive 75% of the co‑payment reimbursed, up to €1,500 per year.

Eye care: up to €370 per year

Up to €120 for glasses and contact lenses. Up to €250 for intraocular lenses or surgical correction of your vision not requiring hospitalisation.

Nutrition and dietary advice: up to €40 per year

If you consult a recognised dietitian, you receive a reimbursement of up to €40 per year.

Travel vaccines: up to €25 per year

When travelling, protection against life‑threatening diseases is important. You receive reimbursement of up to €25 per year for travel vaccines.

Dental care: up to €1800

Receive up to €600 for orthodontics, up to €600 per year for dental prostheses and implants and up to €600 per year for periodontics.

Hearing aid: up to €1200

You’ll receive up to €600 if you buy a monaural hearing aid and up to €1200 for a binaural hearing aid.

Childbirth allowance of 250 euros per birth

When you give birth, CM-MediKo Plan will pay you a lump-sum childbirth allowance.

How much does CM-MediKo Plan cost? 

Your premium depends on your age and the age at which you join. You can easily calculate the exact premium for you and your family online.

If you join CM-MediKo Plan, your dependent children are insured free of charge up to and including the third calendar year following the year of their birth.

Tip: In addition to CM-MediKo Plan, join CM-Hospitaalplan or CM-Hospitaalplan Plus and receive a discount on your hospitalisation insurance premium. That way, you’ll be fully protected.

Frequently asked questions

To join an insurance of CM, you must be a member of CM Vlaanderen and remain so in order to access the services and benefits offered, and you must be in compliance with the CM contribution payment.

You must always take out cover for both yourself and your dependants, except in the following situations:

  • Your dependants are and remain covered by a similar insurance. In that case, you can take out cover for yourself only.
  • You are covered and remain covered by a similar insurance. In that case, your dependants can take out cover for themselves only.

You are entitled to reimbursements after a waiting period:

  • three months for statutory co-payments, travel vaccinations and nutritional and dietary advice; 
  • six months for eye care; 
  • twelve months for periodontology, dental prostheses and implants, orthodontics, hearing aids and the maternity package.

The waiting period begins on the date of enrolment. After the waiting period, you can receive reimbursements from CM-MediKo Plan.

Important: Treatments or costs incurred before or during the waiting period are not eligible.

How to request a refund

Have you received a certificate of treatment from your doctor or healthcare provider? This is the note that you receive after your consultation with your doctor or healthcare provider. You deliver it to us via one of the CM mailboxes. An envelope is not necessary.

You will receive the reimbursement directly into your bank account on four occasions:

  • Costs for which CM has paid the statutory reimbursement between 1 January and 31 March will be reimbursed in mid-June.
  • Costs for which CM has paid the statutory reimbursement between 1 April and 30 June will be reimbursed at the end of August.
  • Costs for which CM has paid the statutory reimbursement between 1 July and 30 September will be reimbursed at the end of November.
  • Costs for which CM has paid the statutory reimbursement between 1 October and 31 December will be reimbursed at the end of February of the following year.

Orthodontics 

Submit the ‘proof of treatment(s) carried out’ or ‘harmonisation document’ issued by the dentist via the My CM app, the CM contact form or a CM mailbox. No envelope is required. The ‘eAttest receipt’ is also a valid supporting document.

For treatments covered by health insurance, the co-payment on RIZIV services is reimbursed within the co-payment guarantee. Did you receive a certificate of treatment? Submit this via a CM mailbox. Your orthodontist can also settle directly with CM. In both cases, the reimbursement of the co-payment will be paid into your account. This is done on a quarterly basis.

For orthodontic treatments not covered by health insurance, a treatment plan is required. This plan includes:

  • The diagnosis 
  • A description of the treatment plan 
  • The appliances to be fitted 
  • The date of issue 

Ask the orthodontist to draw up a treatment plan and submit it. Submit this, together with your first claim, to receive a reimbursement from the CM-MediKo Plan.

You will receive the reimbursement directly into your account. 

Periodontology 

Submit the ‘proof of treatment(s) carried out’ or ‘harmonisation document’ issued by the dentist via the My CM app, the CM contact form or a CM mailbox. No envelope is required. The ‘eAttest receipt’ is also a valid supporting document.

You will receive the reimbursement directly into your account.

If you are entitled to reimbursement from your health insurance, the certificate of treatment provided is also required. Please submit this via a CM mailbox. Your dentist or periodontist can also settle the bill directly with CM. In both cases, the reimbursement will be paid into your account. 

Dental prostheses and implants 

Submit the ‘proof of treatment(s) carried out’ or ‘harmonisation document’ issued by the dentist via the My CM app, the CM contact form or a CM mailbox. No envelope is required. The ‘eAttest receipt’ is also a valid supporting document.

You will receive the reimbursement directly into your account.

For dental prostheses and implants eligible for reimbursement by health insurance, the certificate of treatment provided is also required. Please submit this via a CM mailbox. Your dentist can also settle the bill directly with CM. In both cases, the reimbursement will be paid into your account.

Corrective glasses or contact lenses

Submit the detailed invoice from the optician, stating the prescription and the amount paid, via the My CM app or a CM mailbox. No envelope is required.

If you are entitled to statutory reimbursement, you will also need to provide the certificate of treatment or delivery, together with the prescription.

You will receive the reimbursement directly into your account.

Intraocular lenses and ambulatory surgical correction of your vision not requiring hospitalisation

Send CM the detailed invoice or the application form. Complete it, sign it and have it signed by your doctor or other healthcare provider.

Submit the certificate of treatment provided or of dispensing via the contact form or a CM mailbox. No envelope is required.

You will receive the reimbursement directly into your account. 
 

  1. You only paid your personal contribution to the hearing aid specialist. 
    Your hearing aid specialist will send the certificate of supply to CM. You may also send the detailed invoice to CM. You will receive the reimbursement directly into your account. 
     
  2. You paid the full amount to the hearing aid specialist. 
    You will receive a certificate of delivery yourself. Please submit this via a CM mailbox. You will receive the reimbursement directly into your account. 
     

Submit one of the following documents via the My CM app or the CM mailbox:

  • The BVAC certificate from the pharmacist; 
  • Or the detailed invoice from the hospital or vaccination centre via the My CM app or the CM mailbox; 
  • Or the application form, completed and signed by yourself and the healthcare provider, via the My CM app or the CM mailbox. 

You will receive the reimbursement directly into your account.

You will automatically receive the birth allowance along with the voucher or gift card you choose through the CM baby gift scheme.

Deliver one of the following documents via the My CM app or the CM mailbox:

  • a detailed invoice;
  • the application form (completed and signed by you and the RIZIV-recognized dietician).
  • Did you receive a 'certificate of assistance provided'? Deliver this via the CM mailbox.
