How do I request a refund for a hospital stay?

For the online declaration at CM-Hospitaalplan or CM-Hospitaalplan Plus you do not need a scanner or camera, the declaration is done on the basis of the invoice number. In this video or this info sheet we explain step by step how it works.

• To the online declaration  

Are you unable to submit your declaration digitally? Then print the declaration form , fill it in and submit it signed together with your hospital invoice by e-mail or via a CM mailbox .

If you have them, also submit the invoices below.

• invoices for urgent patient transport (112 transport)
• invoices from the recognized recovery centers Ter Duinen in Nieuwpoort, Hooidonk in Zandhoven and Domaine de Nivezé in Spa
• remaining medical costs after reimbursement by another hospitalization insurance

Submit these documents together with this completed form. Do this by e-mail to [email protected] or via the CM mailbox. 

Please note that CM-Hospitaalplan and CM-Hospitaalplan Plus also provide an automatic reimbursement for the medical costs of examinations directly related to your hospital admission, from 2 month before to 4 months after your admission. Even better insured? For CM-Hospitaalplan Plus, this period is 2 months before to 6 months after your admission.

All remgeld (part of the cost of a visit to the GP you pay yourself) and reimbursable supplements, such as visits to your GP, medicines, follow-up appointments at the hospital or a specialist’s surgery, and laboratory costs, will be reimbursed without you having to take any action. You will, of course, still need to submit medical certificates via the CM-mailbox.

After you have filed your request, you will receive a payment overview and the refund to your account number.

You remain however responsible for the correct payment of the invoice to the hospital.
