Submit medical reports to CM regarding your incapacity for work
The advising physician monitors your incapacity for work. This is done partly on the basis of your medical reports. Here you can read how to submit these to CM.
In My CM
In My CM, go to the 'Incapacity for work' section and select either 'employee/unemployed' or 'self-employed'. Under 'I am incapacitated for work', you will find a button to submit your medical reports.
In the My CM app
In the My CM app, go to 'more' in the menu at the bottom and then to 'upload documents'. Select 'Incapacity for work' and click the button to submit your medical reports.
Why is it important to submit medical records?
- The consulting physician needs these reports to have an accurate and up-to-date picture of your medical condition.
- CM's medical staff does not have access to the online platforms that hospitals, your GP, or other treating physicians or healthcare providers use to exchange reports. Therefore, you must still submit the relevant medical reports to CM separately.
