CM mailbox

Handing in doctor's notes or documents? It can all be done via the CM mailbox. Only a declaration of incapacity for work may not be posted in a CM mailbox, because that must be done digitally or it must be postmarked.

What is allowed in the CM mailbox?

  • application forms
  • certificates ('doctor's notes')
  • ...

Stick a yellow sticker on each document.

You may deposit doctor's notes and other certificates in the mailbox without an envelope. Use a stapler to attach documents that belong together (e.g. certificate for reimbursement of physiotherapy and prescription, doctor's note and referral letter) to each other.

What is not allowed in the CM mailbox?

  • You may not put your declaration of incapacity for work in the CM mailbox. Because your declaration must be made within a certain period, the date on which you make the declaration is important. That date must be clear: either because there is a postmark on it, or because there is a digital trail of the declaration. That trail is not there if you put the declaration in a CM mailbox.
