Global medical record

The global medical record (GMD) shows the bond between the patient and you as a GP and offers many benefits for both.

What are the advantages?

  • Individual patient guidance and consultation between doctors.
  • Better follow-up of vaccinations and screenings by bundling all medical information in one file (examinations, hospitalizations, allergies, etc.).
  • Immediate savings for the patient:
    • pays 30% less co-payment for a consultation or home visit during the day by the GP;
    • pays less co-payment once a year to the specialist if he was referred by the general practitioner via the referral form ;
  • Additional fees for general practitioners.
  • Immediately and automatically a registered therapeutic relationship with the patient.

Billing

Your patient does not have to pay for the opening of a GMD.

Fee to you as a doctor:

  • New GMD : payment within 30 days;
  • GMD extension : automatic payment in February;
    provided that there was contact with the patient within the past two years.

Increased GMD fee

You will receive an increased GMD fee for patients from the defined target group .

This is about patients:

  • between 30 and 85 years;
  • who were given chronic condition status in the previous calendar year.

What amount is involved?

The GMD amount will be increased by 83.33% for these patients.
For 2022, this means a GMD amount of 59.09 euros.

What should you do?

Nothing. When notifying a new GMD relationship or the automatic renewal of existing GMD relationships, CM will automatically pay you the correct GMD rate.

Extend

The GMD is automatically extended in February if you have contact with your patient (consultation or home visit) in the previous two years.

Example

  • 2021: opening GMD (contact with patient)
  • February 2022: automatic GMD grant (because contact in 2021)
  • February 2023: automatic extension GMD (because contact in 2021)
  • February 2024:
    • If also contacted in 2022 or 2023: automatic GMD grant
    • If no more contact in 2022 or 2023: no extension (so no payment)

As a new GP

You can take over a GMD at any time with a notice.

Payment

  • In the opening year:
    The new GP will be transferred the GMD rights. However, the fees are still paid to the first GP.
  • During the renewal year:
    The GP who manages the GMD on December 31 of the year before the extension year receives the fees.

Electronic medical record

The eGMD is the electronic version of the paper GMD. Since 2021, only the electronic version is possible.

For practical questions, please contact your software supplier . For example, they can help you with questions about entering the system or consulting your patient's GMD status.
The software packages approved by the National Intermutualist College can be found on the NIC website .

Applications

Check patient's GMD status

You can query your patient's GMD status via your own software. Within a few seconds you will see whether your patient has a GMD and if so, from which doctor.

This application:

  • is only possible if there is a therapeutic relationship between patient and doctor (i.e. either eID reading in recent months or being a GMD holder);
  • is available for general practitioners, specialists and dentists.

Overview of all your patients with a GMD

You can use your software to request an overview of all patients with whom you have a GMD relationship. The question is automatically sent to all health insurance funds. The electronic response from each health insurance fund contains all your GMD patients.

This application:

  • is only available to general practitioners;
  • is only possible via your own software. An eID does not need to be read for this because it concerns the consultation of all 'your' GMD patients.

Notification of new GMD relationships

If you enter into a GMD relationship with a patient, you communicate this electronically to CM via your software package. Almost immediately you will receive notification whether the GMD has been accepted. If so, payment of this GMD will follow within thirty days .

You will receive an electronic message from CM if another doctor takes over the GMD of one of your patients.

To enter a new GMD relationship:

  • by reading the patient's electronic identity card (eID or kids-ID);
  • or manually for non-electronic cards (isi+ card or SIS card).

Sometimes it is not possible to immediately conclude an eGMD (home visit without a laptop, consultation by a patient who does not have an identity card). Then you can do this afterwards, within seven days after the performance . The entry is then done manually, with the day of the consultation/home visit as the reference date.

GMD in your GP group

A general practitioner group is a group of at least two recognized general practitioners who:

  • working together in the same place ( group practices );
  • or collaborate at different locations within the same GP zone or adjacent GP zones of a GP network ( GP networks ).

The requirements for a GP group include that:

CM bases itself on registration with the Riziv to determine whether or not a doctor is affiliated with a general practitioner group. It is therefore important to properly complete the starting formalities of a group and to report changes in a timely and correct manner.

GMD relationship

The GMD relationship is the one-on-one relationship between doctor and patient. So only one doctor manages the patient's GMD. All doctors within the group have access to the patient files.

The co-payment benefit for the patient applies to all doctors within the group. If you are not the GMD managing physician, but you do invoice the co-payment, you must indicate this.

Other doctors within the group can take over the GMD relationship, without this having consequences for the patient.

Extension GMD

The same general principles apply to the extension of the GMD in a GP group.

Example

GP A and GP B work in the same GP group.

Option 1:

  • 2018: creation of GMD at GP A
  • 2020: consultation with GP B without GMD notification
  • 2021 and 2022: automatic renewal (and payment) GMD with GP A; even if the consultation in 2020 was with GP B

Option 2:

  • 2018: creation of GMD at GP A
  • 2020: consultation with GP B with GMD notification
  • 2021 and 2022: automatic GMD payment to GP B

Leaving or changing a GP group

The co-payment benefit is linked to the GMD-holding doctor .

  • The GMD-holding doctor remains with the general practitioner group: the benefit for the patient applies to all doctors in the group.
  • The GMD-holding doctor leaves the group: he takes the GMD relationship with his patients with him.
    • The patient is no longer entitled to the co-payment benefit with this GP group.
    • Unless another doctor from the GP group takes over the GMD by entering into a new GMD relationship: the co-payment benefit continues to apply to the GP group.
  • The GP is a member of several GP groups: patients who have a GMD relationship with that GP then enjoy the co-payment benefit in all these groups.

General practitioners in training (HAIOs)

General practitioners in training (HAIOs) can open a GMD with their own Riziv number , in the name of the internship supervisor.
However, the GMD relationship continues to exist between the internship supervisor and the patient .

Sumehr

What is it?

Sumehr stands for SUM marized Electronic Health Record .

Sumehr is a concise electronic medical record , which contains all essential health information. It is, as it were, a photo of the patient's health situation at that moment , where you immediately see what is important for the treatment.
Most software packages have the functionality to create, view and modify a sumehr. The layout may differ per package.

What is the goal?

The GP usually creates a sumehr. The file serves to exchange medical data between general practitioners, but also with medical specialists (in a private practice or hospital).
A well-maintained sumehr contains the most recent and most relevant information about a patient. For example, a short-lived flu from four years ago does not belong in a sumehr.
